Is the deploy/jboss-hibernate.deployer in JBoss4.0.2 missing the commons-collections-2.1.1.jar ?

Yes, it was accidentally omitted from the jboss-4.0.2 binary distribution.
You'll need to add it manually to your server/lib directory or upgrade to jboss-4.0.3-RC1.
