If you want to create them, you could do that easily in Guvnor via Desinger. See this video for more info: http://blog.athico.com/2011/07/develop-complex-jbpm-processes-all-in.html.
In the Evaluation example the user krisv is requested to self-evaluate and this is done through a form. Values are constrained by choice lists, through a drop-down and three check boxes. Finally there is a Complete button.
