I'm getting the same problem. Is not possible create a modalPanel with form embeeded in a other form? Is it a restriction or a bug?

It is one of known restriction.
Two rules are very important
a) modal panel must have its own form if it contains form elements
b) modal panel must not be inside the other form if it contains its own
