You should install using features, that installs what is needed and works.
Just type:
features:install camel-hl7
And if you want to see what other features is there type
features:list

thank you for your answer, but it didn't help.
I tried:
JBossFuse:admin@root> features:install camel-hl7
Error executing command: The container is managed by fabric, please use fabric:profile-edit --features camel-hl7/0.0.0 target-profile instead. See fabric:profile-edit --help for more information.
So I did what it asked:
JBossFuse:admin@root> fabric:profile-edit --features camel-hl7/0.0.0 target-profile
JBossFuse:admin@root> features:list | grep hl7
[uninstalled] [2.12.0.redhat-610379 ] camel-hl7 camel-2.12.0.redhat-610379
JBossFuse:admin@root> fabric:profile-list | grep hl7
JBossFuse:admin@root> osgi:list | grep hl7
JBossFuse:admin@root>
what am I missing? Is there some step-by-step tutorial where I can find documentation on how to install camel-hl7 on fuse server?

You are using Fabric, then the idea is to define a profile with what you want such as bundles / features et all. And then assign the profile to container(s).
Try with this if you want to change this on fabric itself, which is assigned to the root container. But you should really learn about the profile concepts and then use that.
fabric:profile-edit --features camel-hl7 fabric
