We have also encountered the same problem, have you been resolved? We used content repository add an unmanaged deployment and publish it to main-server-group, when access http://127.0.0.1:8080/app return forbidden. This problem troubled us for a long time the, and not in the document to find a solution.
I haven't found solutions yet. Currently I'm working to migrate from old Jboss to Wildfly. To keep going, I'm working on standalone mode for now, so I can deploy and test applications. Eventually I'll face this issue and decide if I can use domain mode or not.

Using a directory structure as a deployment will have several problems.
What should trigger a re-deploy?
In domain mode you need to distribute the changes, if you simple change a file you need a process to propagate this consistent to the whole domain. This is more or less not possible.
Therefore the domain mode support only archives deployed with the management api/console
