Dunno how I missed this:
I.e. nested forms are not allowed. Those requirements are applied because of the bug in earlier Internet Explorer versions, as the dialog is moved to the top of the browser DOM tree just after it is rendered. -

2. modal panel should have the form outside
You both agree about "modal panels must have its own form" but has this form to be inside or outside the modal panel? -
